If you're thinking about getting a flat piercing on your skin, such as your collarbone, shoulder blade, forearm, or chest, you've probably come across two terms that sound similar but aren't: microdermal and surface piercing .
Spoiler: they are two different things. And even if at first glance the effect may seem similar, under the skin everything changes. In this article we will look in detail at the difference between microdermal and surface piercing , in order to help you consciously choose the one that is best for you.
In this article I explain the difference well, with clear but professional language. No terrorism, only truth + practical advice from a piercer.
Field experience: what we recommend
In our studio, Pippo has been performing both microdermal and surface piercing for over 10 years , with thousands of applications each year. We know both techniques well, the advantages and problems of each.
Precisely for this reason, Pippo almost always recommends microdermal to clients who want a long-lasting, safe result with as few risks as possible.
On our TikTok and Instagram profiles you can find dozens of real videos with applications, explanations and testimonials about these piercings. If you want to see how we really work, come and take a peek!

What is a Microdermal (or Dermal Anchor)
The microdermal is a single piercing that involves the insertion of a small flat base under the skin (called an anchor), onto which the jewel visible on the surface is screwed.
Main features:
It does not have an entry or exit hole: it is semi-implanted under the skin.
The jewel appears to "peek out of the skin" in a clean and elegant way.
The application can be done with a dermal punch or a special needle.
Advantages:
Stable and well anchored (if done correctly)
Suitable for many areas of the body
Aesthetically minimal and refined
In case of rejection or removal it leaves a very small mark
More impact resistant than a surface piercing
The microdermal is perfect for those who want an original, elegant and unobtrusive piercing, but with as few complications as possible.

What is a Surface Piercing?
The surface piercing is a "classic" piercing with an entry and exit hole, but positioned horizontally or flat on the skin, rather than going through a protruding area (such as the ear or nose).
Main features:
A special paper clip-shaped bar (surface bar) is used
The ends of the jewel are visible, while the bar is under the skin
Often done on the collarbone, pubis, neck, back
Disadvantages:
Increased risk of rejection (the skin tends to push the jewel out)
Slower and more complicated healing
It requires a lot of attention in the post-piercing
If rejected, it can leave visible scars
It is not a piercing to be done lightly. It requires an expert piercer, obsessive care and the awareness that it may not last over time.
Microdermal or Surface? Difference Between Microdermal and Surface Piercing Comparison
I wait | Microdermal | Surface Piercing |
Hole | Single, semi-plant | Double, entry and exit |
Stability | High | Medium-low |
Risk of rejection | Bass | High |
Scars in case of rejection | Minimum | Evident |
Applicable area | Almost everywhere | Selected areas |
Healing | Simpler | Long and delicate |
Aesthetics | Minimal, discreet | More obvious |
Which One Should You Choose?
Ok, now that you understand the technical differences, let's be clear: if you're looking for a piercing that lasts, works and doesn't drive you crazy while healing, the microdermal is the best choice .
It's easier to manage, less stressful to care for, and safer in the long run. Plus, if you ever decide to take it out, it won't leave behind huge holes or ugly scars .
The surface has its charm, of course, but it is much more unstable. It is often rejected, especially if bumped or if the skin is too tight. It is only good in perfect conditions and with a truly experienced piercer.
Frequently Asked Questions (FAQ)
Does microdermal or surface hurt more?
The pain is subjective, but generally the microdermal is quicker and less traumatic. The surface, being a double piercing, can be more annoying.
Which piercing lasts the longest?
The microdermal has a longer average lifespan , especially if positioned correctly and cared for well.
Can I play sports with these piercings?
Yes, but with caution. Microdermal tends to cause fewer problems because it is more stable. Surface can move or get knocked around more easily.
What if it rejects?
In the case of microdermal the mark will be tiny, often invisible after a short while. Surface can leave visible scars , especially if the rejection is slow or inflamed.
Can I change the jewel?
In the microdermal the visible part (top) is changed with a simple screw. The surface is changed like a normal piercing, but only after complete healing.
